As a general rule, we will NOT prescribe controlled substances (stimulants or benzodiazepines like Vallium, Xanax, etc.) via eClinics.

The Medina Clinic is providing eClinic services, via videoconferencing, in which the clinician and the patient are not at the same location. Benefits to eClinics include increased access to care and patient convenience. Potential risks include, but may not be limited to: information transmitted may not be sufficient (poor resolution of video); may not provide for or arrange for emergency care; delays in medical evaluation and treatment; security protocols can fail, causing a breach of privacy; and a lack of access to all the information available in a face to face visit may result in errors in medical judgment.
1) I willl not record any eClinic sessions without the written consent of The Medina Clinic and I understand that The Medina Clinic will not record sessions without my consent;
2) I will inform the doctor if any other person can hear or see any part of our session before the session begins. Likewise, the doctor will inform me if any other person can hear or see any part of the session before the session begins.
3) I understand that I MUST be a resident of Kansas or Missouri to be eligible for eClinic services from The Medina Clinic.
By using eClinic services, you have read, understood and agree to the following consent information regarding eClinic visits: I give my informed consent for the use of online visits in my medical care and authorize The Medina Clinic providers to use eClinics in the course of my evaluation and treatment.
